Our medical services are provided by Beluga Health, a licensed telehealth medical group. Prescriptions are filled by Pharmacy Hub, a US-licensed pharmacy. By using our services, you consent to the data practices described in this policy.

2. INFORMATION WE COLLECT

We collect information that you provide directly, information collected automatically, and information from third-party sources.

PERSONAL INFORMATION YOU PROVIDE:

  • Full name, date of birth, gender, email address, phone number, and mailing address
  • Payment and billing information (credit card numbers, billing address)
  • Government-issued identification when required for identity verification
  • Account credentials (username and password)

PROTECTED HEALTH INFORMATION (PHI):

  • Medical history, current medications, allergies, and health conditions
  • Information shared during telehealth consultations with healthcare providers
  • Prescription records and treatment plans
  • Lab results and diagnostic information

AUTOMATICALLY COLLECTED INFORMATION:

4. HOW WE SHARE YOUR INFORMATION

We do not sell your personal information or protected health information. We may share your information with the following parties:

Beluga Health: Your medical information is shared with Beluga Health providers to deliver telehealth consultations, diagnoses, and treatment plans.

Pharmacy Hub: Prescription and shipping information is shared with Pharmacy Hub for prescription fulfillment and delivery.

Payment Processors: Billing information is shared with third-party payment processors to complete transactions. We do not store full credit card numbers on our servers.

Service Providers: We engage trusted third-party vendors for hosting, analytics, customer support, and email delivery. These providers are contractually obligated to protect your data and use it only as directed.

Legal Requirements: We may disclose information when required by law, subpoena, court order, or government request, or to protect the rights, safety, or property of Major Factors, our users, or the public.

Business Transfers: In connection with a merger, acquisition, or sale of assets, your information may be transferred as part of the transaction.

5. HIPAA COMPLIANCE

To the extent that we create, receive, maintain, or transmit protected health information (PHI) as defined under the Health Insurance Portability and Accountability Act of 1996 (HIPAA), we comply with HIPAA Privacy, Security, and Breach Notification Rules.
