Medical Consent

1. INTRODUCTION

This Medical Consent form ("Consent") is provided by MajorFactors LLC ("Major Factors," "we," "us," or "our") on behalf of Beluga Health, the licensed telehealth medical group that provides clinical services through the Major Factors platform. By proceeding with a telehealth consultation and/or requesting NAD+ therapy through majorfactors.health, you are acknowledging that you have read, understood, and consent to the terms described below.

This Consent covers your agreement to receive telehealth services, your understanding of NAD+ therapy, the known risks and benefits of treatment, and your rights as a patient. Please read this document carefully before proceeding.

2. CONSENT TO TELEHEALTH SERVICES

You consent to engage in telehealth consultations with licensed healthcare providers affiliated with Beluga Health. You understand and agree that:

  • Telehealth consultations are conducted remotely via secure electronic communications, including asynchronous questionnaires and video or messaging platforms
  • The healthcare provider reviewing your case is a licensed physician, nurse practitioner, or other licensed clinician authorized to practice in your state
  • Telehealth services are not a replacement for in-person care in all circumstances, and the provider may recommend in-person evaluation if clinically appropriate
  • You will provide complete, accurate, and truthful information about your medical history, current medications, allergies, and health conditions
  • Providing false or incomplete medical information may result in harm and the provider or Major Factors shall bear no liability for outcomes resulting from inaccurate information you provide
  • You are responsible for disclosing all medications, supplements, and treatments you are currently using

3. NATURE OF NAD+ THERAPY

Nicotinamide Adenine Dinucleotide (NAD+) is a coenzyme found naturally in all living cells. It plays a critical role in cellular energy production, DNA repair, and metabolic regulation. NAD+ levels naturally decline with age, and supplementation via subcutaneous injection has been explored as a means of supporting energy, cognitive function, and cellular health.

You understand that:

  • NAD+ therapy offered through Major Factors is provided as a compounded prescription medication prepared by Pharmacy Hub, a US-licensed 503A compounding pharmacy
  • Compounded medications are not FDA-approved and have not been reviewed by the FDA for safety, efficacy, or manufacturing quality
  • The long-term effects of NAD+ supplementation are still being studied and are not fully established
  • A licensed healthcare provider will evaluate your eligibility based on your health information and may decline to prescribe if it is not clinically appropriate for you

4. POTENTIAL RISKS AND SIDE EFFECTS

As with any medical treatment, NAD+ therapy carries potential risks. Known and reported side effects may include, but are not limited to:

  • Injection site reactions, including redness, swelling, bruising, or discomfort
  • Nausea, headache, fatigue, or dizziness, particularly during initial use
  • Flushing or a sensation of warmth
  • Muscle aches or cramps
  • Gastrointestinal discomfort
  • Allergic reactions, including in rare cases anaphylaxis
  • Unknown or unanticipated risks due to the evolving nature of research on NAD+ therapy

You acknowledge that you have been informed of these potential risks and that no medical treatment is entirely free of risk. You agree to contact your provider or seek emergency care immediately if you experience a severe or unexpected reaction.

5. POTENTIAL BENEFITS

Based on available research and clinical experience, potential benefits of NAD+ therapy may include:

  • Increased cellular energy and reduced fatigue
  • Support for cognitive clarity and mental performance
  • Support for metabolic health and healthy aging processes
  • Improved sleep quality and physical recovery

You understand that individual results vary significantly and that Major Factors makes no guarantee that you will experience any specific benefit from NAD+ therapy.

6. ALTERNATIVES TO TREATMENT

You acknowledge that you have been informed that alternatives to NAD+ injection therapy exist, including but not limited to: oral NAD+ precursor supplements (such as NMN or NR), lifestyle modifications (diet, exercise, sleep), and other wellness approaches. The provider will help you evaluate whether NAD+ therapy or an alternative is appropriate for your individual circumstances.

7. VOLUNTARY PARTICIPATION AND RIGHT TO WITHDRAW

Your participation in NAD+ therapy is entirely voluntary. You have the right to:

  • Decline treatment at any time without penalty or loss of services unrelated to the treatment itself
  • Discontinue treatment after it has begun, understanding that you should consult your provider before stopping
  • Ask questions about your treatment, medications, or alternatives at any time by contacting [email protected]
  • Request a second opinion from an independent healthcare provider

8. PROPER ADMINISTRATION AND PATIENT RESPONSIBILITIES

If a prescription is issued, you agree to:

  • Administer the medication only as directed by the prescribing provider
  • Follow all instructions included with your medication and provided by the provider
  • Store medications as directed to maintain safety and efficacy
  • Not share, sell, or transfer your prescription medication to any other individual
  • Report any adverse effects to the provider promptly
  • Attend follow-up consultations as recommended

9. EMERGENCY DISCLAIMER

The telehealth services provided through Major Factors are not suitable for emergency medical situations. If you are experiencing a medical emergency, including a severe allergic reaction, difficulty breathing, chest pain, or any other life-threatening condition, you must call 911 or go to the nearest emergency department immediately. Do not use this platform to seek emergency care.

10. PRIVACY AND MEDICAL RECORDS

Your health information collected in connection with your telehealth consultation and prescription is considered Protected Health Information (PHI) under HIPAA. It will be collected, used, and disclosed in accordance with our Privacy Policy and Notice of Privacy Practices, available at majorfactors.health. Your information will be shared with Beluga Health providers and Pharmacy Hub only as necessary to provide your care.

11. ACKNOWLEDGMENT AND AGREEMENT

By proceeding with your consultation and/or submitting your intake form, you confirm that:

  • You are at least 18 years of age
  • You have read and understood this Medical Consent in full
  • You voluntarily consent to telehealth consultations and, if prescribed, to NAD+ therapy
  • You have had the opportunity to ask questions and have them answered to your satisfaction
  • All medical and personal information you have provided is accurate and complete to the best of your knowledge
  • You understand and accept the risks, benefits, and alternatives described in this document
